CD1a chimeric antigen receptor (CAR) T cells are an investigational cell-based immunotherapy designed to treat cortical T-cell acute lymphoblastic leukemia (coT-ALL). This therapy involves genetically engineering a patient's T cells to express a chimeric antigen receptor that specifically targets CD1a, a transmembrane glycoprotein exclusively expressed in coT-ALL (approximately 40% of T-ALL cases) and retained at relapse. Unlike pan-T cell antigens, CD1a is absent on mature peripheral T cells and hematopoietic stem cells, which allows for the production of CAR-T cells without the risk of fratricide during manufacturing and minimizes the risk of long-term T-cell aplasia. Preclinical studies have demonstrated robust cytotoxicity against CD1a-positive T-ALL cells, and the therapy is currently being evaluated in Phase 1 clinical trials.
