CD24.28z CAR T cells are an experimental chimeric antigen receptor (CAR) T-cell therapy targeting CD24, a glycosylphosphatidylinositol-anchored surface protein that acts as a "don't eat me" signal to inhibit macrophage phagocytosis. This specific construct incorporates a CD28 costimulatory domain and a CD3ζ activation domain. Developed primarily by researchers at Memorial Sloan Kettering Cancer Center, these CAR T cells are being investigated for the treatment of various hematologic malignancies, including B-cell acute lymphoblastic leukemia, B-cell lymphoma, and acute myeloid leukemia. While showing anti-tumor activity in preclinical models, the therapy faces challenges related to on-target, off-tumor toxicity due to the widespread expression of CD24 on healthy tissues, leading to research into modified signaling domains (such as the 1XX mutation) and pathway modulation to balance efficacy and safety.
