CD25hi Treg depleted DLI is an investigational cell-based immunotherapy developed by the Dana-Farber Cancer Institute for the treatment of relapsed myeloid malignancies following allogeneic hematopoietic stem cell transplantation (HCT). The therapy involves the ex vivo processing of donor lymphocytes, where CD25-expressing regulatory T cells (Tregs) are selectively removed using antibody-conjugated magnetic beads and the CliniMACS system. Tregs are known to suppress anti-tumor immune responses; therefore, their depletion is intended to augment the graft-versus-tumor (GVT) effect of the donor T cells while potentially mitigating the risk of graft-versus-host disease (GVHD). This cell product is typically infused fresh and is being evaluated in Phase 1 clinical trials, often in combination with immune checkpoint inhibitors like ipilimumab, to treat conditions such as acute myeloid leukemia (AML), myelodysplastic syndromes (MDS), and myelofibrosis.
