CD28 blocking antibodies are a class of therapeutic agents designed to selectively inhibit the CD28 co-stimulatory pathway on T cells. Unlike CD28 superagonists, which can trigger a cytokine storm, these blocking agents are typically developed as monovalent antibody fragments (e.g., Fab or scFv) or pegylated formats to avoid receptor cross-linking. By preventing the binding of CD28 to its ligands B7-1 (CD80) and B7-2 (CD86) on antigen-presenting cells, these antibodies suppress T cell activation and proliferation. This mechanism is being explored for the treatment of various autoimmune disorders, such as rheumatoid arthritis and systemic lupus erythematosus, as well as for preventing graft rejection in organ transplantation.
