Overview

A CD3-BCMA bispecific antibody is a T-cell redirecting immunotherapy designed for the treatment of multiple myeloma. This modality features dual binding sites: one specific to the B-cell maturation antigen (BCMA), which is highly and selectively expressed on the surface of malignant plasma cells, and another specific to the CD3 epsilon subunit of the T-cell receptor complex. By cross-linking these two targets, the antibody facilitates the formation of an immunological synapse between cytotoxic T cells and myeloma cells, leading to T-cell activation, cytokine release, and the targeted lysis of the tumor cells independent of MHC-I expression. Several agents in this class, such as teclistamab and elranatamab, have received regulatory approval for relapsed or refractory multiple myeloma.
