CD33.4-1BBz CAR T cells are an investigational chimeric antigen receptor (CAR) T-cell therapy designed to target CD33 (Siglec-3), a transmembrane receptor highly expressed on the surface of myeloid leukemia cells. Developed by researchers at the Baylor College of Medicine, this second-generation CAR construct utilizes an anti-CD33 single-chain variable fragment (scFv) for antigen recognition, linked to a 4-1BB (CD137) costimulatory domain and a CD3-zeta signaling domain. The inclusion of the 4-1BB domain is intended to enhance the metabolic fitness and long-term persistence of the T cells compared to first-generation or CD28-based constructs. This specific CAR-T asset is being evaluated for the treatment of acute myeloid leukemia (AML), both as a standalone agent and as part of combinatorial strategies (e.g., co-transduction with CD123 or CLL-1 targeting CARs) to overcome antigen heterogeneity and prevent leukemic escape.
