Purified CD34+ hematopoietic progenitor cells (HSPCs) are the primary component of a specialized T-cell depleted allogeneic hematopoietic cell transplantation (HCT) regimen developed by Stanford University. This therapeutic approach is designed for patients with advanced hematologic malignancies, including acute myeloid leukemia (AML), acute lymphoblastic leukemia (ALL), and chronic myeloid leukemia (CML). The protocol involves the infusion of highly purified CD34+ stem cells to provide hematopoietic recovery, followed by the sequential or co-infusion of donor-derived regulatory T cells (Tregs) to suppress graft-versus-host disease (GVHD) and conventional T cells (Tcons) to facilitate immune reconstitution and provide a graft-versus-leukemia (GVL) effect. By depleting the initial graft of non-specific T cells and adding back defined subsets, the therapy aims to improve transplant outcomes and reduce treatment-related mortality.
