CD34+ hematopoietic stem cells (HSCs) are multipotent progenitor cells used in the treatment of high-risk non-Hodgkin lymphoma (NHL). In the specific protocol developed by Stanford University, these cells are utilized in a tandem transplantation approach. Patients first undergo autologous hematopoietic stem cell transplantation (auto-HSCT) following high-dose chemotherapy to rescue the bone marrow. This is followed by a non-myeloablative allogeneic hematopoietic stem cell transplantation (allo-HSCT) using donor CD34+ cells. The allogeneic phase employs a unique conditioning regimen consisting of total lymphoid irradiation (TLI) and antithymocyte globulin (ATG), which is designed to promote donor cell engraftment and induce a graft-versus-lymphoma (GVL) effect while minimizing the risk of severe graft-versus-host disease (GVHD).
