CD34+ selected autologous hematopoietic stem cells is an autologous cell therapy used in hematopoietic stem cell transplantation (HSCT) for severe autoimmune diseases, primarily systemic sclerosis (SSc). The therapeutic process involves mobilizing the patient's own hematopoietic stem cells from the peripheral blood, collecting them via apheresis, and performing positive selection for CD34+ cells using immunomagnetic bead technology (such as the CliniMACS system). This selection process enriches the graft for progenitor cells while depleting mature, autoreactive T-cells and other immune effectors. Following high-dose conditioning chemotherapy (typically cyclophosphamide) to ablate the patient's dysregulated immune system, the purified CD34+ cells are reinfused to 'reset' the immune response and promote the development of a new, self-tolerant immune repertoire. This approach is being investigated by institutions like University Hospital Tuebingen to optimize treatment outcomes and reduce treatment-related mortality in patients with progressive systemic sclerosis.
