This cellular therapy consists of CD34+ selected hematopoietic stem cells (HSCs) derived from an allogeneic donor. Developed by Cincinnati Children's Hospital Medical Center (CCHMC), it is specifically designed for pediatric patients with primary immunodeficiency diseases (PID) or bone marrow failure (BMF) syndromes who exhibit waning donor chimerism or graft dysfunction following a previous allogeneic stem cell transplant. The therapy involves the isolation of CD34+ cells from the original donor's mobilized peripheral blood using the CliniMACS system (Miltenyi Biotec) to achieve T-cell depletion, which minimizes the risk of graft-versus-host disease (GVHD). Unlike standard hematopoietic stem cell transplants, this infusion is administered without a preparative conditioning regimen. The primary therapeutic goal is to augment graft function, enhance donor chimerism, and improve immune reconstitution and hematologic parameters such as white blood cell and platelet counts.
