Overview

CD34+CXCR4+ autologous bone marrow cells are a specialized population of progenitor cells harvested from a patient's own bone marrow and selected for the expression of CD34 and CXCR4 surface markers. CD34 is a marker for hematopoietic and endothelial progenitor cells, while CXCR4 (C-X-C chemokine receptor type 4) is the receptor for stromal cell-derived factor 1 (SDF-1), which plays a critical role in the homing of these cells to ischemic or injured myocardium. This cell therapy was investigated in the REGENT trial (NCT00316381) for the treatment of acute myocardial infarction (AMI) in patients with reduced left ventricular ejection fraction. The cells are typically delivered via transcoronary infusion to promote myocardial repair and angiogenesis. Clinical evaluations using intravascular ultrasound have demonstrated that the administration of these cells is safe and does not promote the progression of coronary atherosclerosis, potentially even contributing to a reduction in plaque burden.
