Overview

CD34-selected autologous peripheral blood stem cells is an investigational cell therapy being evaluated for the treatment of severe, treatment-resistant systemic sclerosis (SSc), including juvenile-onset systemic sclerosis (jSSc). Developed by Dr. Paul Szabolcs at the University of Pittsburgh, the therapy involves the collection of a patient's own hematopoietic stem cells via apheresis following mobilization with G-CSF and cyclophosphamide. These cells are then processed to select for the CD34+ population, which serves to enrich for stem cells while depleting autoreactive T and B lymphocytes that drive the underlying autoimmune pathology. Following a high-dose immunoablative conditioning regimen—which may include agents such as alemtuzumab, rituximab, thiotepa, and total body irradiation—the purified CD34+ cells are reinfused. This process aims to 'reset' the patient's immune system, allowing for the reconstitution of a healthy, non-autoreactive immune repertoire.
