CD34 selected hematopoietic stem cells are a cell-based biologic therapy consisting of enriched CD34+ hematopoietic progenitor cells. In the clinical setting investigated by the University of Alabama at Birmingham (UAB), these cells are isolated and enriched ex vivo from donor peripheral blood stem cell grafts using the CliniMACS Prodigy automated immunomagnetic selection system. The selection process utilizes the CliniMACS CD34 Reagent to positively select cells expressing the CD34 surface antigen while simultaneously depleting the graft of T cells, B cells, and NK cells. This depletion is intended to reduce the risk of graft-versus-host disease (GVHD) upon infusion. The therapy is primarily used as a stem cell boost or for re-transplantation in patients with graft failure or low graft function following allogeneic stem cell transplantation for hematologic malignancies, providing hematopoietic reconstitution without the need for intensive pharmacological immunosuppression.
