CD38 CAR KHYG-1 is an experimental, 'off-the-shelf' chimeric antigen receptor natural killer (CAR-NK) cell therapy being developed for the treatment of multiple myeloma (MM) and other CD38-positive hematologic malignancies. The therapy utilizes the KHYG-1 cell line—an immortalized NK cell line derived from a patient with aggressive NK leukemia—which is genetically engineered to express an affinity-optimized CAR targeting CD38, a glycoprotein highly expressed on myeloma cells. The CAR construct incorporates CD28 and CD3ζ co-stimulatory and signaling domains to enhance the cells' expansion and cytotoxic potential. Unlike autologous CAR-T therapies, this approach uses a standardized cell line to provide a ready-to-use product, potentially reducing manufacturing time and costs. Preclinical data indicate that CD38 CAR KHYG-1 cells effectively eliminate MM cells, including those from patients refractory to the anti-CD38 antibody daratumumab, while sparing non-malignant hematopoietic cells with lower CD38 expression.
