CD4 CAR T-cell is an investigational chimeric antigen receptor (CAR) T-cell therapy developed by iCell ImmunityX (formerly iCell Gene Therapeutics) for the treatment of CD4-positive T-cell malignancies, including peripheral T-cell lymphoma (PTCL) and T-cell acute lymphoblastic leukemia (T-ALL). The therapy involves engineering T cells to express a CAR that targets the CD4 antigen, which is highly expressed on the surface of malignant T cells. To overcome the challenge of "fratricide"—where CAR T cells target and kill each other due to their own CD4 expression—iCell utilizes a proprietary non-gene-editing technology to prevent surface expression of CD4 on the therapeutic cells without permanent genomic modification. This allows for the expansion and persistence of the CAR T cells to mediate immune-directed killing of CD4+ tumor cells.
