Overview

CD4-IgG refers to a class of engineered fusion proteins or antibodies that combine the extracellular domain of the human CD4 receptor with the Fc region of human immunoglobulin G (IgG). These molecules are designed to mimic the natural binding site for HIV on T cells and act as decoy receptors or immune modulators. The primary mechanism involves binding to HIV envelope glycoprotein gp120, thereby blocking viral entry into host cells. Some forms have been developed as recombinant fusion proteins for potential use in treating HIV infection by neutralizing free virus and preventing cell-to-cell transmission. Genentech previously developed a dimeric CD4-IgG1 fusion protein for this purpose[3]. Clinical trials have evaluated recombinant human CD4 immunoglobulin G in HIV-infected individuals[2]. The approach is distinct from monoclonal antibodies like ibalizumab, which also target the CD4 receptor but do not use a direct receptor-Fc fusion strategy[5].
