CD4-targeted Tat mRNA-LNP is an experimental latency reversal agent (LRA) designed to reactivate the latent HIV reservoir in CD4+ T cells. The therapeutic consists of mRNA encoding the HIV-1 Tat (trans-activator of transcription) protein, encapsulated within lipid nanoparticles (LNPs) that are surface-functionalized to specifically target the CD4 receptor. Upon delivery and translation, the Tat protein translocates to the nucleus and binds to the HIV-1 long terminal repeat (LTR) promoter, potently inducing viral gene expression. This "shock" phase of the "shock and kill" strategy aims to make latently infected cells detectable by the immune system or susceptible to viral cytopathic effects. In preclinical studies, it has been evaluated in combination with other agents like romidepsin to enhance the reactivation of HIV gene expression in individuals on antiretroviral therapy (ART).
