CD40 antagonist monoclonal antibody is a therapeutic agent designed to inhibit the CD40-CD40L costimulatory pathway by binding to the CD40 receptor on the surface of B cells and other antigen-presenting cells (APCs). This blockade prevents the maturation of APCs and the subsequent activation of T cells and B cells, effectively suppressing both cellular and humoral immune responses. In the context of adeno-associated virus (AAV) gene therapy, developers such as Regeneron are investigating these antibodies to prevent the formation of anti-capsid antibodies—which would otherwise preclude vector re-administration—and to protect transduced cells from T-cell-mediated destruction, thereby enhancing the durability and safety of the therapy.
