CD44v6CAR is a second-generation chimeric antigen receptor (CAR) T-cell therapy designed to target the CD44v6 splice variant, an adhesion molecule overexpressed in various hematological malignancies including acute myeloid leukemia (AML), multiple myeloma (MM), and chronic myeloid leukemia (CML). CD44v6 is implicated in tumor growth, survival, and metastasis of tumor-initiating stem cells. The therapy utilizes T cells engineered to express a CAR typically consisting of an anti-CD44v6 single-chain variable fragment (scFv) linked to costimulatory domains (such as CD28) and the CD3ζ signaling domain. While potent in preclinical models, its development faces challenges due to potential on-target, off-tumor toxicity, as CD44v6 is also expressed on healthy keratinocytes. Research is ongoing to improve its safety profile, including the development of protease-activated versions (sCD44v6CAR) that utilize tumor-specific proteases like MMP-2 to localize activity and mitigate skin toxicity.
