CD45RA depleted donor lymphocyte infusion is a form of adoptive cell therapy in which donor lymphocytes are processed to selectively deplete naïve T cells (CD45RA+), thereby enriching for memory T cells. This approach aims to provide immune protection and enhance immune reconstitution after allogeneic hematopoietic stem cell transplantation (HSCT), particularly from haploidentical or mismatched donors. The rationale is that memory T cells can confer pathogen-specific immunity and anti-leukemic effects while reducing the risk of graft-versus-host disease (GVHD) associated with naïve T cells. The product is typically prepared using immunomagnetic separation technologies such as CliniMACS®. Clinical studies have shown that this strategy improves early post-transplant immune recovery, reduces viral infections (such as CMV, EBV, adenovirus), and has a favorable safety profile with low rates of acute GVHD[1][2][3][6].
