Overview

CD45RA depleted leukocytes are a form of adoptive cell therapy in which donor peripheral blood or stem cell products are processed to selectively remove cells expressing the surface marker CD45RA. This process depletes naïve T cells (CD3+CD45RA+) and enriches for memory T cells (CD3+CD45RO+), while also reducing B and NK cell content to a lesser extent. The resulting product is used primarily in hematopoietic stem cell transplantation (HSCT) settings—either as part of the initial graft or as an addback after transplantation—to promote immune reconstitution and reduce the risk of viral infections without increasing the risk of graft-versus-host disease (GVHD). Mechanistically, this approach aims to transfer pathogen-specific immunity via donor memory T cells while minimizing alloreactivity from naïve T cells that can cause GVHD. Clinical studies have shown that infusions of these products can lead to rapid immune recovery, reduced incidence and duration of viremia from viruses such as cytomegalovirus, Epstein-Barr virus, adenovirus, BK virus, and improved outcomes in patients with high-risk hematologic malignancies or severe infections post-transplant[1][2][5][6][7].
