CD45RA-depleted peripheral blood stem cells represent an engineered hematopoietic stem cell graft designed to improve outcomes in allogeneic stem cell transplantation, particularly in the haploidentical or mismatched setting. The product is manufactured by selectively depleting CD45RA-positive naïve T cells from donor peripheral blood stem cells, typically using immunomagnetic separation technologies like the CliniMACS system. Naïve T cells are the primary mediators of Graft-versus-Host Disease (GvHD) due to their broad repertoire and high reactivity to alloantigens. By removing these cells while preserving CD34+ hematopoietic progenitor cells and CD45RO+ memory T cells, the graft aims to facilitate rapid hematopoietic engraftment and provide immediate, pathogen-specific immunity against viruses like CMV and EBV without the high risk of severe GvHD. This approach is primarily utilized for patients with high-risk leukemia or primary immunodeficiencies who lack a matched donor.
