Overview

CD4CAR is an investigational cell therapy in which immune effector cells (such as T cells or NK cells) are genetically engineered to express a chimeric antigen receptor (CAR) that specifically targets the CD4 protein on the surface of malignant T-cells. The primary mechanism involves redirecting these modified immune cells to recognize and kill cancerous CD4-positive T-cells. This approach is being developed for the treatment of various aggressive hematologic malignancies including peripheral T-cell lymphoma (PTCL), T-cell acute lymphoblastic leukemia (T-ALL), and chronic myelomonocytic leukemia (CMML). Preclinical studies have demonstrated that both CAR-T and CAR-NK versions targeting CD4 can effectively eliminate tumor cell lines and patient-derived samples in vitro and in vivo, with enhanced cytotoxicity compared to unmodified donor-derived natural killer or double-negative T-cells. The therapy has received orphan drug designation from the FDA for PTCL[1][3][7].
