CD5-specific CAR-T cells are engineered T cells designed to recognize and kill cells expressing the CD5 antigen, which is present on the surface of normal and malignant T cells. These therapies introduce a chimeric antigen receptor (CAR) directed against CD5, enabling targeted cytotoxicity against CD5-positive malignant cells, especially in T-cell malignancies such as T-cell acute lymphoblastic leukemia (T-ALL) and mature T-cell lymphoma. To prevent fratricide (self-killing of CAR-T cells due to shared CD5 expression), modifications such as genetic editing (e.g., CRISPR knockout of CD5 in donor T cells or downmodulation of CD5 protein) are often used, enhancing persistence and reducing toxicity. These products are typically autologous or allogeneic cell therapies, developed for patients with relapsed/refractory T-cell malignancies with poor prognosis, and are under clinical investigation for other CD5-positive hematologic malignancies.
