CD6-CAR Tregs are allogeneic regulatory T cells genetically engineered to express a chimeric antigen receptor specific for the T‑cell differentiation antigen CD6, designed to enhance antigen-directed immunosuppression and restore immune homeostasis in settings such as chronic graft-versus-host disease after allogeneic hematopoietic cell transplantation.[1][2] The CAR construct redirects Tregs to CD6-expressing cells, where engagement blocks CD6-mediated costimulatory pathways, downregulates effector T-cell activation and differentiation, and reduces production of pro‑inflammatory cytokines while leveraging the intrinsic suppressive functions of FoxP3+ Tregs to promote tolerance and limit autoimmunity.[1][2][4] CD6-CAR Tregs are being developed as a living cell therapy to induce durable, antigen-specific immune tolerance, potentially enabling long-term control of cGVHD and related transplant or autoimmune pathology with reduced need for systemic immunosuppression.[1][2]
