CD7-targeted CAR-T cell therapy is an investigational modality of adoptive cell therapy designed to treat T-cell malignancies, such as T-cell acute lymphoblastic leukemia (T-ALL) and T-cell lymphoblastic lymphoma (T-LBL). These therapies involve engineering T cells to express a chimeric antigen receptor (CAR) that recognizes the CD7 antigen, which is highly expressed on malignant T cells. A significant challenge in developing CD7 CAR-T cells is 'fratricide,' where the CAR-T cells attack each other because they also express CD7. To mitigate this, developers use various strategies such as CRISPR/Cas9 gene editing to delete the endogenous CD7 gene, protein blockers to sequester CD7, or 'natural selection' of CD7-negative T cells. Notable candidates in this category include Wugen's WU-CART-007, which is an allogeneic, off-the-shelf product.
