CD7.PEBL-CART is an autologous chimeric antigen receptor T cell therapy engineered to target CD7-expressing cells, especially for the treatment of relapsed or refractory T-cell acute lymphoblastic leukemia (T-ALL) and T-cell lymphoblastic lymphoma. The therapy uniquely combines an anti-CD7 CAR construct (41BB-CD3ζ) with a CD7 protein expression blocker (PEBL) to prevent CD7 expression on the CAR T-cell surface, thereby avoiding T-cell fratricide—an issue which impedes CAR T cell manufacturing for T-cell malignancies.[1][2][4][5][6] CD7 is highly and consistently expressed in both leukemic blasts and normal T-cells, making it a suitable but challenging target. By intracellularly sequestering CD7 using PEBL technology, the therapy allows for effective expansion and persistence of engineered CAR T cells. Clinical studies, primarily at Bambino Gesù Children’s Hospital (Rome) and through collaboration with MediSix Therapeutics, suggest the therapy induces minimal residual disease (MRD)-negative complete responses with an acceptable safety profile in patients with relapsed/refractory T-ALL.
