CD74-M-GB is an experimental antibody-drug conjugate (ADC) designed to target CD74-expressing malignant B cells, particularly in diffuse large B-cell lymphoma (DLBCL) and chronic lymphocytic leukemia (CLL). It consists of a monoclonal antibody specific for CD74 (HLA class II histocompatibility antigen gamma chain) conjugated via a maleimide linker to M-GB, a modified version of the broad-spectrum cathepsin inhibitor GB111-NH2. CD74 and Cathepsin L (CTSL) are often co-overexpressed in DLBCL, and their high expression is associated with poor prognosis. Upon binding to CD74 and internalization, the ADC releases the M-GB payload, which inhibits cysteine cathepsins (including CTSL). This inhibition triggers apoptosis and caspase-3 activation, effectively killing the lymphoma cells. CD74-M-GB is being investigated as a potential therapy for relapsed or refractory DLBCL and CLL, particularly in cases where standard treatments like rituximab have failed.
