Overview

CD74-RNase immunotoxin (2L-Rap-hLL1-γ4P) is a recombinant biologic immunotoxin developed by Immunomedics for the treatment of CD74-positive hematologic malignancies. The drug is a fusion protein consisting of two molecules of ranpirnase (Rap), an amphibian ribonuclease derived from *Rana pipiens* eggs, fused to the N-termini of the light chains of hLL1 (milatuzumab), a humanized IgG4 monoclonal antibody targeting CD74. CD74 is a cell-surface molecule highly expressed on B-cell lymphomas, multiple myeloma, and certain solid tumors. The mechanism of action involves the antibody component binding to CD74-positive cells, followed by internalization of the complex. Once inside the cell, the ranpirnase payload is released into the cytosol, where it degrades RNA and inhibits protein synthesis, ultimately leading to cell death. Preclinical studies demonstrated significant antitumor activity in human lymphoma xenograft models, including Burkitt lymphoma.
