CD8 depleted donor lymphocyte refers to a cellular therapy product consisting of peripheral blood or leukapheresis-derived lymphocytes from a human allogeneic (usually unrelated or related) donor that have been selectively depleted of CD8+ T cells. The primary rationale is to reduce the risk of graft-versus-host disease (GVHD) while preserving the beneficial graft-versus-leukemia (GvL) effect mediated mainly by other immune cells, particularly CD4+ T cells. This product is used as an adjunctive immunotherapy in patients with hematologic malignancies—most notably chronic myelogenous leukemia (CML), myelodysplastic syndromes (MDS), and secondary acute myeloid leukemia (sAML)—who relapse after allogeneic stem cell transplantation or are refractory to standard therapies. The mechanism involves providing immunologic "help" via activated donor CD4+ T cells that can reverse recipient immune exhaustion and promote anti-tumor responses without engraftment, thus minimizing GVHD risk[1][2][3][6]. Clinical studies show this approach can induce remission with lower rates of severe GVHD compared to unmanipulated DLI[1][6].
