CEA-targeted ISAC (Immune-Stimulating Antibody Conjugate) is a therapeutic candidate being developed by Bolt Biotherapeutics using its proprietary Boltbody platform. The molecule consists of a monoclonal antibody specific for Carcinoembryonic Antigen-Related Cell Adhesion Molecule 5 (CEACAM5/CEA) conjugated to a next-generation Toll-like receptor 7/8 (TLR7/8) agonist payload. This approach is designed to convert "cold" tumors into "hot" tumors by activating the innate immune system within the tumor microenvironment. Upon binding to CEA-expressing tumor cells, the ISAC promotes antibody-dependent cellular phagocytosis (ADCP) and activates tumor-infiltrating myeloid cells, such as macrophages and dendritic cells. This activation leads to the production of pro-inflammatory cytokines (e.g., IL-12, IFNγ, TNFα) and the subsequent priming of a durable, systemic T-cell-mediated anti-tumor response. Preclinical studies in colorectal, pancreatic, and lung cancer models have demonstrated significant tumor growth inhibition, complete regressions, and the development of immunological memory.
