Overview

This antibiotic regimen, evaluated by Sunnybrook Health Sciences Centre, is used for the prevention of surgical site infections in patients undergoing ear surgeries such as cochlear implantation, stapedotomy, and tympanoplasty. The protocol involves a combination of cefazolin, a first-generation cephalosporin administered intravenously at induction, and a subsequent one-week oral course of amoxicillin and clavulanic acid. Cefazolin and amoxicillin function by inhibiting bacterial cell wall synthesis through the targeting of penicillin-binding proteins (PBPs), while clavulanic acid serves as a beta-lactamase inhibitor to protect amoxicillin from enzymatic degradation by resistant bacteria. This specific sequential treatment strategy is being investigated in a Phase 4 clinical trial to compare its efficacy against a standard single-dose preoperative antibiotic prophylaxis.
