Overview

The combination of cefazolin and probenecid is a therapeutic regimen used primarily for treating moderate-to-severe cellulitis and other skin and soft tissue infections. This combination leverages probenecid's ability to inhibit renal tubular secretion of beta-lactam antibiotics like cefazolin, thereby increasing and prolonging cefazolin's serum concentrations[1][2]. ## Pharmacological Properties Cefazolin is a first-generation cephalosporin antibiotic administered intravenously, while probenecid is an oral medication that competitively inhibits renal tubular secretion of certain drugs. When combined, probenecid increases plasma levels of cefazolin by reducing its renal clearance[2]. This advantageous drug interaction allows for once-daily dosing of cefazolin, which would otherwise require multiple daily administrations to maintain therapeutic levels. The typical dosing regimen consists of cefazolin 2g intravenously once daily plus probenecid 1g orally once daily[1][4]. This combination has been shown to maintain therapeutic serum concentrations of cefazolin over a 24-hour period, with median antibiotic trough concentrations of approximately 2.35 μg/mL[1]. ## Clinical Efficacy Clinical studies have demonstrated that cefazolin + probenecid is effective for treating moderate-to-severe cellulitis and other skin and soft tissue infections. In a randomized, double-blind equivalence trial comparing cefazolin (2g IV) plus probenecid (1g orally) with ceftriaxone (1g IV) plus placebo, clinical cure rates at the end of treatment were 86% for the cefazolin-probenecid group and 96% for the ceftriaxone-placebo group[1][4]. At one month follow-up, cure rates were maintained at 96% and 91%, respectively[1]. This combination has also been successfully used in the treatment of methicillin-sensitive Staphylococcus aureus (MSSA) bacteremia, particularly in patients with chronic kidney disease[2]. ## Advantages and Considerations The cefazolin + probenecid regimen offers several advantages: 1. Cost-effectiveness: It's described as a "cheap, practical, and effective treatment option"[1]. 2. Once-daily administration: Convenient for outpatient and home-based therapy[1][2]. 3.
