The combination of cefazolin and probenecid is a therapeutic regimen used primarily for treating moderate-to-severe cellulitis and other skin and soft tissue infections. This combination has been shown to maintain therapeutic serum concentrations of cefazolin over a 24-hour period, with median antibiotic trough concentrations of approximately 2.35 μg/mL[1]. ## Clinical Efficacy Clinical studies have demonstrated that cefazolin + probenecid is effective for treating moderate-to-severe cellulitis and other skin and soft tissue infections. In a randomized, double-blind equivalence trial comparing cefazolin (2g IV) plus probenecid (1g orally) with ceftriaxone (1g IV) plus placebo, clinical cure rates at the end of treatment were 86% for the cefazolin-probenecid group and 96% for the ceftriaxone-placebo group[1][4]. At one month follow-up, cure rates were maintained at 96% and 91%, respectively[1]. This combination has also been successfully used in the treatment of methicillin-sensitive Staphylococcus aureus (MSSA) bacteremia, particularly in patients with chronic kidney disease[2]. ## Advantages and Considerations The cefazolin + probenecid regimen offers several advantages: 1. Cost-effectiveness: It's described as a "cheap, practical, and effective treatment option"[1]. 2. Once-daily administration: Convenient for outpatient and home-based therapy[1][2]. 3. Antimicrobial stewardship: Avoids the need for third-generation cephalosporins in most patients[1]. Side effects are generally similar to those of other antibiotic regimens, though nausea has been reported to be more common with cefazolin-probenecid compared to ceftriaxone[1]. This combination represents a valuable option for outpatient parenteral antibiotic therapy, particularly in hospital-in-the-home settings[3], and may be especially useful for patients with renal insufficiency or those requiring once-daily treatment due to insurance or logistical constraints[2].
