Cefazolin + bacitracin + cefalexin is an investigational multimodal antibiotic prophylactic regimen studied in the PADIT Pilot trial (NCT01002911) for the prevention of cardiac implantable electronic device (CIED) infections. The regimen consists of three distinct components: a pre-procedural intravenous dose of cefazolin, an intraoperative pocket wash with bacitracin, and a post-procedural course of oral cephalexin. Cefazolin and cephalexin are first-generation cephalosporins that inhibit bacterial cell wall synthesis by binding to penicillin-binding proteins (PBPs), while bacitracin is a polypeptide antibiotic that interferes with the dephosphorylation of undecaprenyl pyrophosphate, a critical step in peptidoglycan synthesis. This combination targets common Gram-positive pathogens, including methicillin-resistant organisms, associated with device infections. The regimen was evaluated by the Population Health Research Institute in high-risk patients undergoing arrhythmia device procedures but did not show a significant reduction in infection-related hospitalizations compared to standard single-dose cefazolin.
