Overview

It is effective mainly against Gram-positive bacteria and some Gram-negative organisms[3][5]. - **Metronidazole** is a nitroimidazole antibiotic that disrupts DNA synthesis in anaerobic bacteria and certain protozoa, resulting in cell death. It provides excellent coverage against anaerobic pathogens commonly found in the gastrointestinal tract[1][4][5]. - **Gentamicin** is an aminoglycoside antibiotic that binds to the 30S ribosomal subunit of susceptible bacteria, inhibiting protein synthesis and causing bacterial cell death. It has potent activity against many aerobic Gram-negative bacilli[2][4][5]. This combination regimen offers comprehensive coverage for both aerobic (including Gram-positive cocci and Gram-negative rods) and anaerobic organisms encountered during clean-contaminated or contaminated surgical procedures such as colorectal surgery, gynecological surgery (e.g., hysterectomy), appendectomy, or other abdominal operations[1][4][5]. The use of all three agents together may be reserved for high-risk cases or when maximal spectrum prophylaxis is needed.
