This is a multi-component formulation used in Tumescent Anesthesia Antibiotic Delivery (TAAD), primarily studied for the prevention of surgical site infections (SSI) and biofilms in colorectal surgery. The solution consists of two antibiotics—cefazolin (a first-generation cephalosporin) and metronidazole (an anaerobic-active nitroimidazole)—dissolved in a tumescent lidocaine anesthesia (TLA) base. The TLA base typically contains lidocaine (local anesthetic), epinephrine (vasoconstrictor to prolong local effect and reduce systemic absorption), and sodium bicarbonate (buffer to reduce injection pain) in physiologic saline. By delivering high concentrations of antibiotics directly into the subcutaneous tissue, TAAD aims to achieve superior local tissue levels compared to standard intravenous administration, potentially reducing the risk of incisional infections.
