Cefbuperazone is a second-generation cephamycin antibiotic developed by Toyama Chemical (now Fujifilm Toyama Chemical). As a member of the cephamycin class, it features a 7-alpha-methoxy group on the cephalosporin nucleus, which confers significant resistance to hydrolysis by many beta-lactamases. The drug is administered parenterally via intravenous or intramuscular injection. Its mechanism of action involves the inhibition of bacterial cell wall synthesis by covalently binding to penicillin-binding proteins (PBPs), which are essential transpeptidases required for the cross-linking of peptidoglycan chains. Cefbuperazone exhibits broad-spectrum activity against Gram-positive and Gram-negative bacteria, with particular potency against anaerobic bacteria and certain Gram-negative rods. It is primarily used in Japan for the treatment of respiratory tract infections, urinary tract infections, and intra-abdominal infections such as peritonitis and cholecystitis.
