This is a combination of three broad-spectrum β-lactam antibiotics—cefepime, meropenem, and piperacillin-tazobactam. Each agent acts by inhibiting bacterial cell wall synthesis through binding to penicillin-binding proteins (PBPs), leading to cell lysis and death. Cefepime is a fourth-generation cephalosporin with activity against both Gram-positive and Gram-negative bacteria. Meropenem is a carbapenem antibiotic effective against many multidrug-resistant organisms, including Pseudomonas aeruginosa. Piperacillin-tazobactam combines an extended-spectrum penicillin (piperacillin) with the β-lactamase inhibitor tazobactam, expanding its spectrum to include many β-lactamase–producing organisms. This combination would be considered in rare clinical scenarios for severe or resistant infections where maximal empiric coverage is required[1][2][5].
