cefetamet pivoxil

Cefetamet pivoxil is an orally active, third-generation cephalosporin antibiotic. It is a prodrug of the active metabolite cefetamet (Ro 15-8074). Following oral administration, the pivoxil ester is rapidly hydrolyzed by esterases in the intestinal wall and liver to release active cefetamet into the systemic circulation. Its mechanism of action involves the inhibition of bacterial cell wall synthesis by binding to one or more **penicillin-binding proteins (PBPs)**, which in turn inhibits the final transpeptidation step of peptidoglycan synthesis in bacterial cell walls, leading to cell lysis and death. Cefetamet exhibits broad-spectrum activity against various Gram-positive and Gram-negative bacteria, being particularly potent against common respiratory pathogens such as *Streptococcus pneumoniae*, *Haemophilus influenzae*, and *Moraxella catarrhalis*. It was originally developed by Roche and is primarily indicated for the treatment of respiratory tract infections, including sinusitis, pharyngitis, and pneumonia, as well as urinary tract infections.
