Cefoperazone is a semisynthetic, broad-spectrum third-generation cephalosporin antibiotic. It is used to treat a variety of bacterial infections in different parts of the body, including the respiratory tract, abdomen, skin, and female genital tracts. Cefoperazone works by binding to specific penicillin-binding proteins (PBPs) inside the bacterial cell wall, inhibiting the final stage of bacterial cell wall synthesis and leading to cell lysis through autolytic enzymes. It is particularly notable for its activity against *Pseudomonas aeruginosa* and other Gram-negative bacteria that are often resistant to other cephalosporins. The drug contains an N-methylthiotetrazole (NMTT) side chain which can cause hypoprothrombinemia and disulfiram-like reactions with alcohol due to inhibition of vitamin K epoxide reductase and aldehyde dehydrogenase[1][2][4][5][6].
