Cefotaxime + gentamicin is a combination of two antibiotics used primarily for the treatment of severe bacterial infections, including those caused by Gram-negative and some Gram-positive organisms. Cefotaxime is a third-generation cephalosporin that inhibits bacterial cell wall synthesis by binding to penicillin-binding proteins, leading to cell lysis and death. Gentamicin is an aminoglycoside antibiotic that binds to the 30S subunit of the bacterial ribosome, causing misreading of mRNA and inhibition of protein synthesis, which results in bactericidal activity. The combination may exhibit additive or synergistic effects against certain resistant bacteria such as Escherichia coli, Klebsiella pneumoniae, and Pseudomonas aeruginosa[2][3]. This regimen is sometimes considered in neonatal sepsis or other serious infections where broad-spectrum coverage is required; however, there are risks such as increased nephrotoxicity when these drugs are used together[1].
