Ceftazidime-avibactam + metronidazole is a combination therapy used primarily for the treatment of complicated intra-abdominal infections (cIAI). Ceftazidime is a third-generation cephalosporin antibiotic that exerts its bactericidal effect by binding to essential penicillin-binding proteins, thereby inhibiting bacterial cell wall synthesis. Avibactam is a non–beta-lactam beta-lactamase inhibitor that protects ceftazidime from degradation by inactivating certain beta-lactamases produced by resistant bacteria. Metronidazole is an antimicrobial agent effective against anaerobic bacteria and protozoa, working through DNA strand breakage and inhibition of nucleic acid synthesis. The combination provides broad-spectrum coverage against aerobic gram-negative pathogens (including multidrug-resistant strains) as well as anaerobes, making it suitable for severe abdominal infections. This regimen has demonstrated efficacy comparable to meropenem in phase 3 clinical trials for cIAI[1][2][4][5][6].
