This triple combination may be considered in rare cases where broad-spectrum coverage against Gram-positive, Gram-negative, atypical organisms (including *Mycoplasma*, *Chlamydia*, *Neisseria gonorrhoeae*), and resistant pathogens is required—for example in severe community-acquired pneumonia with risk factors for multidrug resistance or complicated pelvic inflammatory disease when standard regimens fail or are contraindicated[2][3][5]. However, this specific triple regimen does not appear as a standard recommended therapy in major guidelines; more commonly used combinations include ceftriaxone plus doxycycline (with or without metronidazole) or monotherapy with moxifloxacin[3][6].
