Overview

This is a combination of four distinct medications: ceftriaxone (a third-generation cephalosporin antibiotic), meropenem (a carbapenem antibiotic), rivaroxaban (a direct factor Xa inhibitor anticoagulant), and vancomycin (a glycopeptide antibiotic). Each component has its own mechanism of action. Ceftriaxone and meropenem both inhibit bacterial cell wall synthesis but through different mechanisms and with different spectrums of activity. Vancomycin prevents incorporation of N-acetylmuramic acid and N-acetylglucosamine-peptide subunits into the peptidoglycan matrix of Gram-positive bacterial cell walls. Rivaroxaban selectively inhibits factor Xa, interrupting the blood coagulation cascade. This specific four-drug combination is not a standard formulation and would typically represent concurrent administration of separate medications for complex clinical scenarios.
