Ceftriaxone + amoxicillin + clavulanic acid + dicloxacillin is a multi-antibiotic regimen evaluated in the CHILD@HOME_BJI trial (NCT04563325) for the treatment of uncomplicated bone and joint infections, such as osteomyelitis and septic arthritis, in children. The regimen involves the use of ceftriaxone, a third-generation cephalosporin administered intravenously, followed by or compared against oral antibiotics including co-amoxiclav (amoxicillin and clavulanic acid) for children under five years of age and dicloxacillin for children aged five years and older. All components are beta-lactam antibiotics that inhibit bacterial cell wall synthesis by binding to penicillin-binding proteins (PBPs), while clavulanic acid acts as a beta-lactamase inhibitor to prevent enzymatic degradation of amoxicillin. The trial, conducted by Rigshospitalet in Denmark, aimed to determine if an oral-only strategy is non-inferior to the standard initial intravenous therapy.
