CEL-4000 is a peptide-based therapeutic vaccine developed for the treatment of rheumatoid arthritis. It is designed to modulate the immune response by promoting antigen-specific regulatory T cell activity rather than broad immunosuppression. The drug incorporates the PG70 epitope from cartilage proteoglycan aggrecan conjugated with the DerG LEAPS immune cell binding ligand (ICBL). This conjugate specifically targets CD4+ T cells, leading to an increase in regulatory CD4+ FoxP3+ T cells and a decrease in pathogenic pro-inflammatory responses. Preclinical studies have shown that CEL-4000 can inhibit disease progression and shift the immune environment from pro-inflammatory to anti-inflammatory in animal models of rheumatoid arthritis. The mechanism involves modulation of cytokine production and rebalancing adaptive immune homeostasis without weakening overall immune defense against infections or cancer[1][3][5][8].
