Celecoxib + cetuximab is an investigational combination therapy consisting of two distinct agents with complementary mechanisms of action. Celecoxib is a selective cyclooxygenase-2 (COX-2) inhibitor classified as a nonsteroidal anti-inflammatory drug (NSAID), while cetuximab is a recombinant chimeric human/mouse IgG1 monoclonal antibody targeting the epidermal growth factor receptor (EGFR). The rationale for combining these drugs lies in their ability to inhibit mutually reinforcing pathways involved in tumorigenesis. Preclinical studies have shown that this combination can synergistically suppress cell proliferation, migration, invasion, and induce apoptosis in various cancer models including oral squamous cell carcinoma and colorectal cancer. Mechanistically, the combination inhibits EGFR signaling (including downstream PI3K/AKT pathway), reduces prostaglandin E2 production, decreases VEGF expression, alters β-catenin subcellular distribution, and diminishes cancer stem cell populations. Clinical trials have explored this regimen primarily in refractory colorectal cancer; however, results have been mixed with some studies terminated early due to insufficient efficacy[1][2][4][6].
