This is a combination regimen consisting of two immune checkpoint inhibitors—**cemiplimab** (a high-affinity, fully human IgG4 monoclonal antibody that targets PD-1/programmed cell death protein 1) and **fianlimab** (a fully human IgG4 monoclonal antibody targeting lymphocyte activation gene-3, LAG-3)—plus standard cytotoxic chemotherapy (most commonly a platinum-doublet in the context of non-small cell lung cancer). Both immunotherapies act by releasing inhibitory brakes on the immune system: cemiplimab targets PD-1, enhancing T cell-mediated anti-tumor activity, while fianlimab targets LAG-3, another inhibitory receptor on immune cells. The addition of chemotherapy is thought to further potentiate immunogenic cell death, facilitating a more robust anti-tumor immune response. The combination is under investigation primarily for advanced non-small cell lung cancer (NSCLC) and other advanced malignancies, with emerging data from phase 2/3 randomized controlled trials[1][3].
