Overview

Cephalothin + gentamicin is a combination of two antibiotics used together for the treatment of serious bacterial infections, particularly in hospitalized or immunocompromised patients. Cephalothin is a first-generation cephalosporin antibiotic that inhibits bacterial cell wall synthesis by binding to penicillin-binding proteins, leading to cell lysis and death. Gentamicin is an aminoglycoside antibiotic that binds to the 30S subunit of bacterial ribosomes, inhibiting protein synthesis and resulting in bactericidal activity. The combination provides broad-spectrum coverage against both Gram-positive (mainly via cephalothin) and Gram-negative (mainly via gentamicin) bacteria. This regimen has been used empirically for severe infections such as sepsis, febrile neutropenia in cancer patients, and other life-threatening conditions where mixed or resistant organisms are suspected[1][3][4][5]. However, this combination carries a significant risk of nephrotoxicity (acute tubular necrosis), especially at high doses or in elderly patients or those with pre-existing renal impairment[1][2]. Careful monitoring of renal function is required during therapy.
